Notice of Absence due to Medical Reason(s)
Students who miss class due to a medical emergency are asked to provide the Center for Student Accessibility with medical documentation within 10 business days of the event for review and verification. Accommodations most often requested include the ability to make up assignments/exams and to not penalize an attendance grade during the day(s) missed. The course instructor has the discretion to determine what, if any, accommodations are possible in their course.
Documentation can be submitted to the CSA via email at accessibility@citytech.cuny.edu and should include:
- Student's full name.
- Date of incident/seen by medical professional.
- Information on when student can return to school.
- In some cases, information supporting why the medical issue prohibits the student from attending classes.
- Contact information for the medical practice.
- The practice stamp and/or medical license number.
The CSA will not provide notification of absence for a scheduled doctor's appointment, or for the medical issues faced by a person other than the student.
Medical documentation or information received by the CSA will not be shared with others. Notification of Absence letters will only reference a medical issue, it will not state a diagnosis, illness or treatment plan. Students are free to share their medical information with whomever they choose, but cannot be mandated to share their medical conditions with faculty. For any long-term medical conditions, students should contact the Center for Student Accessibility in person, by phone, or email to discuss further supports.
